    Job Opportunity – HIM Specialist

    Ensure chart integrity and completion within specified guidelines.  Performs release of information according to state and/or HIPAA standards. Perform charge reconciliation and charge validation.

     

     

    Responsibilities

      

    Essential Functions

    • Upholds the values and mission of the Tandem Hospital Partners family
    • Maintains patient confidentiality
    • Analyzes medical record to identify deficiencies and assigns deficiencies to the correct provider.
    • Manages medical record deficiencies to ensure chart completion according to standards.
    • Communicates with health care providers to keep them informed of chart availability and deficiency status
    • Insures that all record keeping and information disbursement complies with local policies, federal and state laws
    • and statutes and HIPAA regulations
    • Appropriately and accurately pulls records for patient care, quality review, physician review, committee review and audits in a timely manner.
    • Observe confidentiality and safeguards all patient related information.
    • Responsible for coordinating the release of medical information to insurance companies, lawyers, state, and federal agencies.
    • Responsible for processing of subpoenas and court orders, at the direction of the HIMS director/manager.
    • Verify authorizations in accordance with hospital policy and procedures and state and federal laws. Ensure that all requests for records are stamped with date received and logged in the correspondence log book.
    • Performs charge reconciliation and collects information for identifying operational trends within charge capture.
    • Manages outstanding and/or incorrect charges for each medical record.
    • Maintain a good working relationship within the department and other departments.
    • Adhere to hospital requirements, policies, and standards.
    • Provide excellent customer service.
    • Answering main phone line in the department.
    • Complete annual education requirements in relation to the job function
    • Maintain patient confidentiality at all times
    • Maintain regulatory requirements, including all state, federal and local regulations
    • Comply with all organizational policies and standards regarding ethical business practices
    • Communicate the mission, ethics and goals of the organization
    • Participate in performance improvement and continuous quality improvement activities
    • Attend regular staff meetings
    • Perform other duties as assigned

     

     

    Qualifications

     

    Minimum Qualifications

    • High school graduate or equivalent
    • 2 years’ hospital medical records experience preferred
    • Candidate must possess a working knowledge of HIPPA
    • Medical anatomy, physiology and terminology preferred
    • Working knowledge of CPT and ICD-10 codes
    • Basic Computer skills including Microsoft Suite

    Knowledge, Skills and Abilities

    • Knowledge of health information management services.
    • Knowledge of documentation regulations of JCAHO and CMS
    • Knowledge of ICD-10, CPT, HCPCS and HIPAA requirements
    • Strong organizational and interpersonal skills.
    • Ability to determine appropriate course of action in more complex situations.
    • Ability to work independently, exercise creativity, be attentive to detail, and maintain a positive attitude.
    • Ability to manage multiple and simultaneous responsibilities and to prioritize scheduling of work.

    Physical Requirements/Working Conditions

    Work is performed during regular work hours, but will require overtime and/or weekend hours to meet deadlines.

    Position requires light to moderate work with up to 50 pounds’ maximum weight to lift and carry.

    Position requires reaching, bending, stooping, and handling objects with hands and/or fingers, talking and/or hearing, and seeing.
